WHIE Community Care Plans Pilot

In 2011, MSMC pledged $25,000 to fund the Wisconsin Health Information Exchange Network (WHIE) Community Care Plan pilot. Through a task force of the EDCC that focused on community care plans, WHIE developed a model for MCOs to expand its recorded member data to incorporate continuity of care documents, including current medications. The model is intended to establish a standard for other MCOs to follow in an effort to promote patient safety.
